The Algoma OHT is a new partnership announced by the Minister of Health in July, 2020, with the intention to work together to achieve a shared vision of providing a continuum of integrated health, social, and health promotion services in Algoma.
Integrated care means that:
Our vision is to develop an integrated health and social system in which people receive seamless, effective care where and when they need it. Focusing on the unique needs of Algoma residents, we want to involve and collaborate with patients (including individuals who have experience across health and social systems), caregivers, and family members.
We are looking to YOU for advice and partnership to develop new solutions to large issues in our community, such as how to support healthy ageing and manage conditions better in the community.
