Algoma Diabetes Education & Care

A collaborative team of Certified Diabetes Educators, both registered dietitians and registered nurses, a social worker, foot care nurse and primary care providers, utilize current clinical practice guidelines to assist both adults and children with Type 1, Type 2, Gestational Diabetes and those with Prediabetes.

Group Health Centre Cardiac Rehab

This program is designed to work collaboratively with physicians and other allied health professionals to provide an individualized exercise program, education and counselling using a holistic approach. Physician/Nurse-Practitioner referral is required.

This team of RNs works closely with Primary Care Providers to support patients with multiple chronic illnesses and Congestive Heart Failure in their home. This program is for GHC rostered patients only.

Provide safe, effective and efficient anticoagulation to maintained on Warfarin utilizing a clinic coordinated by a specialized anticoagulation RN through medical directives. This program is for GHC rostered patients only.
