Group Health Centre: Algoma Diabetes Education & Care

ADEC is an interactive, interdisciplinary team for Type 1, Type 2, gestational and pediatric diabetes, and IGT. Services offered include Individual and Group sessions for newly diagnosed/ongoing follow-up.

Group Health Centre Cardiac Rehab

This program is designed to work collaboratively with physicians and other allied health professionals to provide an individualized exercise program, education and counselling using a holistic approach. Physician/Nurse-Practitioner referral is required.

Group Health Centre: Congestive Heart Failure

GHC’s Congestive Heart Failure (CHF) program was created to improve outcomes for patients with CHF by improving the care between the hospital and home.This program is for GHC rostered patients only.

Group Health Centre: Anticoagulation Program

The Anticoagulation Clinic is designed to care for cardiac patients prescribed an anticoagulant such as Warfarin (Coumadin). The clinic is staffed by registered nurses who specialize in the care and management of patients who are taking anticoagulants. This program is for GHC rostered patients only.

Cardiac Rehab is and an interdisciplinary team that provides education, support and an exercise program for those patients who have experienced a Cardiac Event in the last calendar year.

The Algoma Respiratory Education Program is a service provided by GHC to the people of the Algoma District who have or are at risk of developing respiratory problems such as; asthma, chronic obstructive pulmonary disease (COPD), or who need screening for lung disease.

At GHC, our Registered Dietitians are trained to provide excellent advice to patients of all ages on optimizing their fitness, enhancing digestive health, managing chronic disease, and improving overall wellness and vitality through personalized nutrition. This program is for GHC rostered patients only.

Nurses with foot care management certification provide an examination, treatment, and education regarding the prevention of disease and conditions of your feet.

Group Health Centre: HARP (HIV & AIDS Resource Program)

Provide services to people living with, at risk and affected by HIV, as well as provide case management, support and crisis intervention. Work on the promotion of Harm Reduction Strategies in the community.
