As we launch ourAnnual Plan for 2025-26, we are pleased to mark the beginning of our sixth year as the AOHT — a network built on collaboration, trust, and a shared commitment to improving the health and wellness of the Algoma District.
Through consultation and co-design, we are proud to reintroduce our 2024-27 Logic Model to you. This logic model is a visual representation of our program goals and objectives, built with shared understanding with our partners. It was first introduced in 2024-25 to serve as our plan for the next three years, with updates to be made to the activities each subsequent year. Our renewed activities are found below.
