As we launch our Annual Plan for 2026-27, we are pleased to mark the beginning of our seventh year as an OHT. We are a network built on collaboration, trust, and a shared commitment to improving the health and wellness of the Algoma District.
Our 2026–27 logic model is a visual representation of our goals and activities, created with collaborative commitment from our partners. We first introduced our logic model in 2024–25 as a plan for three years. Each year, we update the activities to reflect our progress and priorities.
