Early Frailty Identification
Background
The Algoma region has a significantly higher proportion of older adults compared with the rest of the province. In Algoma, almost 30% of our population is over the age of 65 and 51% of our population is over the age of 50. These numbers are projected to increase significantly in the coming years. Provincial Geriatrics Leadership Ontario (PGLO) estimates that roughly 1 in 4 adults aged 65 and older in the Algoma region are living with frailty. This number is expected to continue to rise in the coming years, and will likely be exacerbated by overall decreased access to primary care in the region.
Frailty is a common condition among older adults, and early identification allows for timely interventions that can significantly improve their quality of life, reduce hospitalizations, and enhance overall healthcare outcomes. Frail older adults were first identified as a year one priority population for the Algoma OHT. To date, much of the population health management work of the OHT has been focused on improving care for frail older adults.
