Healthy Aging Advisory Committee

Background

Healthy aging and care for frail older adults were identified as a priority

of the AOHT in its initial 2020 application. To advance this work and support

coordinated, population-based approaches to care, the AOHT established

the Healthy Aging Advisory Committee to guide strategy, collaboration,

and system planning for older adults across the region.

Key Objectives

The Healthy Aging Advisory Committee will identify and prioritize

healthy aging initiatives based on community need and population

health data, while championing the expansion of key initiatives such as

early frailty identification and healthy aging education sessions. The

committee will also support coordination and alignment across related

planning tables and working groups to foster collaboration, collective

advocacy, and efficient use of resources while overseeing the implementation

of initiatives and change ideas outlined in the AOHT Healthy Aging Strategy.
